From my personal observation it works like this: The number of fans in your profile is only updated when a new follower 'joins'. It is not updated when accounts unfollow you or get deleted by the Newgrounds administration.
If two accounts unfollow and a new one follows you, it can look as if you lost one - instead of gaining one.
That being said, there is one exception to this: If you manually block followers ( https://www.newgrounds.com/account/blocked ) the counter gets updated immediately.
(I do this every now and again. If you just want to 'remove' an account as a follower without permanently blocking them you can just block and immediately unblock them.)
